Patient Arrival Policy
For your protection, and that of others, all dogs must be on a leash and properly controlled while in the waiting area or exam rooms.
All cats must be presented in an appropriate cat carrier or on a leash.
Please let us know if you need assistance with your pet.
New Clients: arriving 10-15 minutes early will help expedite paper work and allow our doctor to spend more time with your pet! It will also help your pet adjust to our staff and clinic. Filling out a New Client Registration Form (in .pdf format) will help speed up your check in process. Reminders
For your convenience, reminder letters will be mailed to you when your pet is due for vaccinations or routine preventive health care.
Address/Name/Phone
Please help keep our records accurate by updating us with any address, name, or phone number changes. If you notice inaccurate information within your file, please notify us. We depend on current, accurate information to insure you receive your pet's reminders, callbacks, and help us communicate with you better!

Although at this time, none of us at the Animal Hospital of Sun Prairie fluently speaks a second language, we appreciate the fact that in today's world, many of you do.
